Free PIAA Athletic Physical

School of the Future Athletics

When: June 4th, 2024 from 9 am – Noon

Where: School of the Future

Greetings Parents, Guardians, and Students:

On June 4th the University of Pennsylvania is sending a team of licensed physicians to evaluate our students and complete the PIAA Physical Form needed to participate in athletics during the 2024-25 school year.  There is NO COST for this service.

Students must bring the completed CIPPE packet with them on June 4th.   Completing this form serves as permission for students to be evaluated by the physicians from Penn.

Section 6 should be left blank and will be completed by the physicians here at school.

Again, once this is completed students will be cleared to participate in Athletics for all of next school year.

Please contact Mr. Jones, Athletic Director, at cwjones@philasd.org with any questions/concerns.
